No, I just know one was issued with every A5 scope sold in WWI to the Marines and Army and I just don’t know if I have ever seen one.

Probably not; not only were they easily lost of course, but unless the steel was hardened, I suspect the blades would easily twist. They were enclosed in the boxes of every scope sold, but the one in Campbell’s book is the only one I’ve ever seen.
